Your first appointment will consist of a consultation in which Dr. Babiuk will diagnose you and provide you with treatment options. Depending on the severity of the case, surgery may be performed on the same day as your consultation. However, if you are on a complex treatment plan or have a complicated medical history, you will likely require a thorough evaluation during your first appointment and treatment during your second appointment.
Online Patient Registration
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
Before your consultation, please take the time to visit our Patient Forms page, where you can print and fill out the forms we will need from you at your first appointment. Additionally, please bring:
- Your referral slip
- Your ID
- Any X-rays (if applicable)
- If you have medical or dental insurance, please bring the necessary completed forms.
Please note that patients under 18 years of age must be accompanied by a parent or guardian during the consultation.
Additionally, if you will be undergoing IV anesthesia during your treatment, please make sure not to eat or drink anything for eight hours prior to surgery and to bring an adult to drive you home.
If you have a medical condition that may be of concern during surgery, such as high blood pressure, diabetes, etc., or if you are taking any medications, please make sure to alert our office prior to your surgery.
At The Centre for Oral Surgery in Joliet, we accept Visa, MasterCard, American Express, Discover and CareCredit. Payment is due at the time service is rendered unless other arrangements have been made in advance. We are in network with most major insurance providers and make every effort to provide you with convenient financial options. Most insurance companies respond within one month—please call us if your monthly statement does not reflect your insurance payment within that timeframe.
